Elevator Accident To Blame
In Death At 49ers Stadium Construction Site
by Bay City News / June 11, 2013 12:41
pm
Construction on the San
Francisco 49ers’ new stadium in Santa Clara has been halted after a worker was
killed in an elevator accident at the site early this morning. The accident was
reported at 6:53 a.m. at the site of what will be Levi’s Stadium, the $1.3
billion venue at Centennial Boulevard and Tasman Drive slated to open in 2014.
Robert Rayborn, project co-director with general contractor Turner-Devcon, said
the worker who died was an employee of subcontractor Schindler Elevator Corp.
He said the accident occurred in one of 14 elevator shafts at the stadium site
but that the circumstances remain under investigation. The worker’s name has
not yet been released. “We have 1,000 employees a day working there and we have
very few accidents, especially serious ones,” Rayborn said. “We can’t describe
our sadness. This is such a tragedy.”He said the California Division of
Occupational Safety and Health is investigating the accident. In the meantime,
work on the stadium has been stopped and workers have been sent home. Project
co-director Jonathan Harvey said it is not clear when work will resume, but
that the site is shut down at least for the day. Harvey said safety is the
company’s primary concern. “We will never put a worker in a hazardous
situation,” he said. A statement released by Turner-Devcon this morning read,
“We are deeply saddened to confirm that a worker died this morning while
working on the stadium construction site. Our thoughts and prayers go out to
his family during this very difficult time.” The statement said that in
addition to Cal/OSHA, the Santa Clara police and fire departments are looking
into the accident, and that Turner-Devcon will also conduct its own
investigation. Grief counselors will be made available to friends and coworkers
of the victim in the coming days and weeks, according to the company. Santa
Clara city spokesman Dan Beerman said word of the death has sent a shock wave
through the city. “Everybody’s walking around here in a daze,” he said. Jeff Burbank, Bay City News

GEORGE TOWN: An uncompleted
flyover connecting to the still-under-construction second Penang bridge at Batu
Maung collapsed at around 7pm Thursday, burying at least one car and a
motorcycle under the debris. Police fear that four people are dead after one
car, a white-coloured Perodua Kelisa was found buried under the rubble. State
Fire and Rescue Department confirmed that one motorcyclist, who was earlier
trapped under the debris, has been pulled out. He suffered injuries to his
head. Hundreds of firemen, volunteers and medical personnel from throughout the
state have been mobilised to the scene. Firemen are removing debris to reach
the cars and motorcycles under the rubble, comprising mostly metal scaffolding
and steel beams. Bayan Baru MP Sim Tze Tzin, who was on the scene, said about a
30m-long span of the ramp was believed to have collapsed around 7pm, as work to
install concrete boulders to strengthen the structure was ongoing. He said he
was informed a few people may be trapped under the rubble. State executive
councillor Lim Hock Seng said the cause of the collapse had yet to be
determined. "We cannot confirm the cause of the accident," he said,
adding that a full investigation would be carried out. Police have began
deploying traffic policemen as the incident has caused a severe jam to ensue
near the Tun Dr Lim Chong Eu Expressway. Many motorists
have also stopped to look at the debris. As of 9:30pm Thusrday night, fire
department authorities have briefed Sim and Lim. R. Thilasheni, 24, whose car
windscreen was shattered by a falling beam during the incident, was still in a
state of shock. "We were lucky to come out alive. We were only about
three seconds away from when the ramp collapsed," she said. She was travelling
with her friend G. Gajashaantini, also 24. She believed a motorcyclist was
trapped under the rubble and said a beam fell onto a car in the incident. The
24km-long bridge was set for completion next month. The longest bridge in
South-East Asia and 20th in the world was supposed to go through road
commissioning and testing in August before being officially opened in
September.

Man dies in excavator accident at Thomson Rd
construction site.

QIUYI /06 JUNE 2013
SINGAPORE — A man has died in a construction site
on Thomson Road, trapped inside a toppled excavator. The accident happened
early this morning (June 6) and authorities have ordered all work at the site
to stop. When rescuers arrived, the excavator was half buried in debris and the
operator’s body trapped inside. Recovery work was hampered because the
operator’s cabin was pinned by a concrete slab. A crane was used to stabilise
the heavy machine, before rescue work could go on. Mr Daniel Seet, ground commander
at Singapore Civil Defence Force, said: “We had used another excavator with
hydraulic cutter attached to slowly cut away the concrete beams and flooring
that were pressing against the driver’s cabin, and once that was done
successfully we were able to go in, do some further fine cutting to gain access
into the cabin and remove the casualty.” Paramedics pronounced the victim, Mr
Low Chim Lam, dead at 1.40pm. He was a 59-year-old Singaporean. According to
the Manpower Ministry’s preliminary findings, Mr Low was operating the
excavator on the first storey when the floor gave way. The excavator toppled
and fell to the basement level, partially buried by rubble, with him trapped
inside.Family members of the deceased were spotted arriving at the scene in the
afternoon, visibly distraught. Channel NewsAsia understands Mr Low leaves
behind children, a wife and elderly parents. Mr Low’s cousin, Mr Loh Chin Seng,
said: “I already informed my uncle, his parent. Now they are waiting at home.
But this morning they, his father already watched the television, they saw some
construction... The excavator collapsed but he still didn’t know it was his
son. But now he knows, I’ve informed him.” The work site is on a busy stretch
of Thomson Road, and used to be a row of shops and apartments known as Novena
Ville. Workers have been tearing down the building for the past few months for
redevelopment into a four-storey commercial and residential project called
Novena Regency. Authorities have ordered all work at the site to stop and the
Manpower Ministry is investigating the accident. CHANNEL NEWSASIA
